FORMAL COMPLAINT PROCESS
This policy outlines procedures for patients to submit formal complaints regarding privacy violations, treatment concerns, billing disputes, staff conduct, or accessibility issues, in accordance with applicable federal and state regulations.

● All complaints will be acknowledged within five (5) business days
● Investigations will be conducted promptly and confidentially
● Written responses will be provided within thirty (30) days
● No retaliatory action will be taken against complainants
Filing a complaint will not affect your access to dental care or treatment. This practice prohibits retaliation against patients who exercise their rights under applicable privacy laws.
